Our
office is a specialty practice. If you belong to an HMO or any
plan that requires prior authorization from your primary care
doctor, you must bring a referral form and co-payment with you every time you have a visit. Without
a referral, we may need to reschedule your appointment. |
If
your medical condition requires hospitalization or surgery,
our surgery coordinator will assist you with the necessary
paperwork and precertification
requirements. |
